To apply for any one of the following authorities, complete and submit the appropriate certificate or license application (Forms OA 141, OA 150 or OA 151) and the additional documents listed. A separate OA 210 must be completed for each business official listed on your application. You must submit a separate application for each authority type you wish to obtain.

If your application is approved, you will receive notification to file proof of liability insurance before the operating authority certificate can be issued. (Insurance does not apply to brokers.)
Note: The certificate/license application process for operating authority involves multiple steps and pieces of information and requires the applicant’s continuing involvement and cooperation with DMV staff. It is critical that all required information is current and submitted in a timely manner. If after 90 days, you have failed to respond to a request for information, DMV may cancel your application. If your application has been canceled and you later decide to reapply for operating authority, you will need to begin the process as a new applicant.

All permit applicants must file proof of minimum limits of liability insurance at the time of application. Employee Hauler and Taxicab applicants are also required to submit a $50 filing fee with the application.
