Minors must present documentation to show their age and relationship to their guardian. Appropriate documentation will come from list A or a combination of documents from list B and C.
The following documents are acceptable proof of age and relationship on their own provided the last names or addresses on them are the same as the ones stated on the guardian’s identification: State Issued ID Passport Driver’s License/Permit *The last names or addresses must match on the minor and adult’s identification. If they do not, the minor must have a combination of documentation from list B and C.
The following documents are acceptable proof of age and must be accompanied by documentation in List C: State Issued ID Passport Driver’s License/Permit School ID
The following documents are acceptable proof of relationship to the guardian and must be accompanied by documentation in List B: Birth Certificate *If the minor or guardian’s name has changed and no longer matches the way it is presented on the Birth Certificate, supplemental Marriage Certificate, Divorce papers, or Court Order of Name Change are required Adoption Papers Power of Attorney giving full guardianship without limitation
For minors 4-13 years of age, the minor does not need a photo ID. The guardian’s photo ID and the minor’s birth certificate are all that is required, provided that the guardian’s name is on the birth certificate. If the guardian is not a birth parent, Adoption Papers or Power of Attorney giving full guardianship without limitation is required.
